Job Summary
Travel Physical Therapist in an outpatient setting in Columbus, IN. Responsibilities include evaluating patients, developing and implementing individualized treatment plans, directing discipline-specific therapy services to support patient return to community activities, maintaining licensure, completing required documentation, and participating in quality improvement and student education activities. Requires Indiana PT license, outpatient PT experience, and BLS if required; travel-based role with housing/relocation reimbursements where applicable.
Required Qualifications
- Bachelor's, Master's, or Doctoral degree in Physical Therapy from an accredited program recognized by the APTA
- Active Indiana Physical Therapy license required to start the assignment
- Minimum of relevant outpatient physical therapy experience; new graduates are not eligible for this role
- BLS (Basic Life Support) certification from AHA or ARC may be required
- Demonstrated competency in documentation, goal writing, letters of medical necessity, and charge entry in compliance with regulatory and payer requirements
- Ability to evaluate patients, develop treatment plans, and modify plans based on clinical reasoning and functional goals
- Willingness to participate in student supervision and professional development activities
