Job Summary
Provide high-quality outpatient physical therapy services to improve patient mobility and function. Participate in care plans for patients with acute injuries and chronic conditions. Collaborate with interdisciplinary teams to ensure continuity of care and measurable outcomes. Maintain documentation and compliance with state and facility standards.
Required Qualifications
- Bachelor’s, Master’s, or Doctoral degree in Physical Therapy from an accredited program approved by the APTA
- Active Illinois Physical Therapy license required to start the assignment
- BLS certification from AHA or ARC may be required
- Experience in outpatient physical therapy preferred
- Experience with musculoskeletal and orthopedic diagnoses preferred
- Prior travel or per diem clinic experience preferred
- Experience with electronic medical records preferred
