Physical Therapist - Travel Contract

Job Summary
Provide skilled physical therapy in patients' homes to improve function, mobility, and quality of life within a Monticello, IA home-health program. Deliver in-home evaluations and develop individualized treatment plans, collaborate with interdisciplinary teams, and document care in the electronic medical record. Shift is 0800-1600, 40 hours per week with potential occasional float to nearby service locations; travel required within the region. Requires Physical Therapy licensure in Iowa (Bachelor’s/Master’s/Doctoral degree in Physical Therapy) and BLS certification; Epic EMR experience preferred. Travel contract with comprehensive benefits including housing support where applicable, travel/license reimbursement, and referral bonuses.
Required Qualifications
- Active Iowa Physical Therapist license
- Bachelor's/Master's/Doctoral degree in Physical Therapy from an APTA-approved program
- BLS certification from AHA or ARC
- Home health experience preferred
- Experience with electronic medical records (Epic preferred)
