Job Summary
Physical Therapist travel contract performing home health assessments and skilled interventions in the Hardinsburg, KY area. Providers will develop and implement treatment plans to improve mobility, endurance, pain management, and functional independence, documenting patient progress to support regulatory compliance. Role supports collaboration with interdisciplinary home health teams, coverage across Elizabethtown-Hardin County and Hardinsburg-Meade County, and involves travel between patients’ homes with mileage reimbursement provided between visits.
Required Qualifications
- Master's or Doctoral degree in Physical Therapy from an accredited program
- Active Kentucky Physical Therapy license required to start the assignment
- Previous home health experience required, including use of HCHB electronic medical record
- BLS certification from AHA or ARC may be required
- Experience providing patient assessments, developing home-based treatment plans, and documenting progress in home health settings
