Job Summary
Travel Physical Therapist position in Lakeside, AZ on a 13-week contract with a 32-hour weekly guarantee. Responsibilities include providing physical therapy services to geriatric patients in a skilled nursing facility, documenting treatments, and developing individualized care plans within an interdisciplinary team. Requires a PT degree (Bachelor's/Master's/Doctoral) and an active Arizona PT license, with BLS potentially required. Schedule is days, 4x8 (Mon–Fri). Travel/relocation benefits and housing options may be available.
Required Qualifications
- Bachelor's, Master's, or Doctoral degree in Physical Therapy from an accredited program approved by the APTA
- Active Arizona State Physical Therapy license required to start the assignment
- BLS (Basic Life Support) certification from American Heart Association or American Red Cross may be required
- Experience working in a skilled nursing facility or long-term care setting is required
- Experience providing care to geriatric patient populations, including restorative and maintenance programs
- Demonstrated ability to document skilled treatments, develop individualized care plans, and work within interdisciplinary teams
