Physical Therapist - Travel Contract

Job Summary
Provide goal-directed physical therapy to geriatric residents to improve mobility, function, and quality of life in a traveling assignment at a skilled nursing facility in Mentor, OH. Coordinate with an interdisciplinary team, document interventions in the NetHealth/PCC EMR, contribute to restorative and discharge planning efforts, and support continuity of care within a 100+ bed facility. Responsibilities include applying PT clinical expertise to enhance mobility, functional independence, and overall resident well-being while ensuring compliance with standards of care and regulatory requirements.
Required Qualifications
- Bachelor’s, Master’s, or Doctoral degree in Physical Therapy from an accredited program approved by the APTA.
- Active Ohio physical therapy license required to start the assignment.
- BLS certification from American Heart Association or American Red Cross may be required.
- Experience working with older adults/geriatrics in skilled nursing or long-term care environments preferred.
- Experience using NetHealth/PCC or similar skilled nursing EMR preferred.
