Physical Therapist - Travel Contract

Job Summary
Provide home-based physical therapy services in Muskegon, MI. Deliver assessments, individualized treatment plans, and progress documentation to improve patient function and safety. Collaborate with interdisciplinary home health team members to support transitions and continuity of care. Travel contract with full medical benefits, 401(k) matching, housing options, travel and license reimbursement, and recruiter support. Shift 0800-1700, Monday–Friday, with holiday rotation; no weekend coverage per client notes. Home health experience and EPIC electronic medical record use are preferred, with emphasis on independent work in patients' homes.
Required Qualifications
- Bachelor's/Master's/Doctoral degree in Physical Therapy
- Active Michigan physical therapy license
- BLS certification from AHA or ARC
