Job Summary
Deliver physical therapy services to patients recovering from injuries and managing chronic conditions within Wabash, Huntington, and Grant County, Indiana. Maintain Monday through Friday coverage from 8:00 AM to 4:30 PM, with weekend rotations every third Saturday and potential holiday availability. Ensure compliance with Medicare guidelines for home care while utilizing HomeCare HomeBase proficiency. Hold an active, unencumbered Indiana Physical Therapy license, BLS certification, and CPR certification, and provide reliable transportation to support the assignment.
Required Qualifications
- Bachelor's, Master's or Doctoral Degree in Physical Therapy from accredited schools approved by the APTA
- Active State License is Required to Start the Assignment
- Current unencumbered Physical Therapy license for the state of Indiana
- Recent home care experience proficiency
- HomeCare HomeBase proficiency
- Understanding of Medicare guidelines for home care
- CPR certification
- Driver's license, vehicle insurance, and dependable vehicle
- Weekend requirement: Every 3rd Saturday
- May require availability on observed holidays
