Job Summary
Physical Therapist – Travel Contract needed to provide home health PT in Pierce County, WA. Responsibilities include evaluating patients in home health settings, developing and implementing plans of care, documenting according to state and CMS requirements, supervising aides/PTAs as allowed, completing progress notes and discharge summaries, and traveling between patient visits across Pierce County and nearby service areas. Schedule is Monday–Friday, 08:00 AM–04:00 PM with a weekly guaranteed 36 hours and a productivity target (30 points). The role requires willingness to travel to various sites within Pierce County and possibly float to nearby service areas; shift is days; office-free travel to patient homes is expected.
Required Qualifications

- Doctoral degree in Physical Therapy from an accredited program approved by the APTA
- Current Physical Therapy licensure in Washington state or a compact license with privilege to practice in Washington
- Current CPR certification
- Valid driver's license, vehicle insurance, and reliable vehicle for patient visits
- Experience providing home health physical therapy, including patient evaluations, development of plan of care, and CMS documentation
- Ability to complete progress notes per agency policy and prepare discharge summaries at required intervals
- Willingness to travel throughout Pierce County for patient visits and to cover other nearby agency service areas as needed
- Must meet productivity targets (30 points) and a weekly hour guarantee of 36 hours
- Minimum education requirement: Bachelor’s Degree
- Preferred: experience with HomeCare HomeBase electronic documentation
