Job Summary
Plan and conduct segments of treatment programs to restore physical, social, and mental functions while serving as a co-leader in interdisciplinary patient groups. Provide home health physical therapy services with documentation in HCHB and completion of OASIS. Assist patients in recovering from injuries and managing chronic conditions. This travel contract requires a Doctor of Physical Therapy with an active license and at least one year of home health experience. Candidates must hold AHA BLS or CPR certification, a valid driver's license, and be able to travel locally to patient homes. The role offers a Monday through Friday, 5 x 8 schedule with no weekends or call, including 100% paid housing and weekly pay.
Required Qualifications
- Doctor of Physical Therapy (DPT) degree required if licensure was obtained on or after January 2016; or foreign equivalent.
- Active Physical Therapy license in the state of practice required.
- Minimum 1 year of home health physical therapy experience required.
- Must have knowledge and experience with HCHB and OASIS.
- Valid Driver's License and at least 21 years of age required.
- AHA BLS or AHA-CPR certification required.
- Cardiopulmonary Resuscitation (CPR) or Basic Life Support certification required.
- Must be able to travel locally and drive to patient homes.
- Mileage paid per government rate.
Desired Qualifications
- Experience working in home health settings beyond one year preferred.
