Job Summary
Travel Physical Therapist in home health across service areas; plan, implement, and evaluate individualized PT care plans in patients' homes; collaborate with an interdisciplinary team; educate patients and families; responsibilities include patient assessment, durable medical equipment recommendations, and documentation of plans of care and progress notes; requires ability to travel locally and float as needed within the client’s service area; includes comprehensive benefits, housing options, and travel/license reimbursement.
Required Qualifications
- Bachelor of Science in Physical Therapy from an APTA-approved program, or Master’s or Doctoral degree in Physical Therapy
- Current state license as a Physical Therapist
- CPR certification
- Current driver’s license and transportation
- 1-2 years of PT experience in acute care or rehabilitation setting
- Ability to travel locally and work a flexible schedule
- Fluent English
