Job Summary
Provide in-home physical therapy in a travel-capable role, planning, implementing, and evaluating individualized PT care plans across the lifespan. Work within the interdisciplinary team, educate patients and families, document plans of care and progress notes, assess patients, recommend DME, and adapt equipment for home environments. Travel locally within the service area; duties require patient assessment, documentation, and ongoing education for safe transfer techniques and use of assistive devices. Required Qualifications
- Bachelor of Science in Physical Therapy from an APTA-approved program, or Master’s or Doctoral degree in Physical Therapy
- Current state license as a Physical Therapist
- CPR certification
- Current driver’s license, reliable transportation, and insurance
- 1-2 years of experience as a Physical Therapist in an acute care or rehabilitation setting
- Ability to travel locally and work a flexible schedule
- Fluent English (read, write, speak)
- Experience with durable medical equipment and ability to order/adapt rehabilitative equipment for home environments
