Job Summary
Travel Home Health Physical Therapist in Hughesville, MD providing in-home PT to adults and older adults recovering from injury or managing chronic conditions on a structured weekday schedule. Typical schedule is Monday–Friday, ~8:00 AM–4:30 PM, with a local travel radius of ~10–15 minutes between visits. Responsibilities include delivering skilled physical therapy, maintaining clinical autonomy, documenting in MatrixCare EMR, and coordinating care across Southern Maryland counties. Strong emphasis on managing a mobile caseload, achieving patient-centered outcomes, and contributing to community-based home health services.
Required Qualifications
- Degree in Physical Therapy (Bachelor’s, Master’s, or Doctoral) from an accredited program approved by the APTA
- Active Maryland physical therapy license or ability to obtain state licensure prior to start
- Basic Life Support (BLS) certification from AHA or ARC may be required
- Home health clinical experience preferred
- Comfort with a mobile caseload, clinical autonomy, and rapid learning in community/home settings
- Experience using MatrixCare EMR preferred
