Physical Therapist - Travel Contract

Job Summary
Travel Physical Therapist to provide outpatient rehabilitation and contribute to patient recovery. Responsibilities include evaluating patient function, developing goal-specific treatment regimens, submitting written plans of care, scheduling frequency and duration of treatments, and educating patients and caregivers regarding condition, prognosis, and treatment rationale. Delegates responsibility to Physical Therapist Assistants and aides, coordinates care with physicians and interdisciplinary staff, and ensures adherence to facility policies and physician orders. Shift schedule is 8:00 am–5:30 pm Monday–Thursday and 8:00 am–12:00 pm Friday (40-hour workweek, no weekend requirement). Primarily outpatient with acute care as secondary. Required Qualifications
- Bachelor‘s, Master‘s, or Doctoral degree in Physical Therapy from an accredited program approved by the APTA
- Active Michigan Physical Therapist license as stated by LARA (current and verifiable)
- Active American Heart Association BLS certification required
- Demonstrated outpatient physical therapy experience required
- PT experience documented on work history and skills checklist, current to within one year
- Two supervisory reference forms completed within the last two years
Additional Requirements
- Candidates must reside outside the facility‘s 50-mile radius per client restriction. Local candidates within that radius will not be considered.
