Physical Therapist - Travel Contract

Job Summary
Screen, evaluate, and rehabilitate patients with physical functioning disorders and wound care while following regulatory and clinical practice standards. Perform standardized assessments, develop and document plans of care, and record treatment minutes per requirements. Provide patient, family, and caregiver education, collaborate with interdisciplinary team members, and utilize evidence-based interventions including manual techniques, therapeutic exercise, and ambulation training. Supervise PT assistants, rehabilitation aides, and students as needed. Participate in patient care conferences, utilization review, and rehabilitation meetings. Document all clinical information in the electronic record and provide comprehensive discharge summaries. Work 8-hour days in a skilled nursing facility in Purcell, OK, with weekend requirements waived.
Required Qualifications
- Bachelor's, Master's, or Doctoral degree in Physical Therapy from an accredited program approved by the APTA
- Active Oklahoma state PT license
- Minimum 1 year of PT experience
- Level II clinical experience beyond Level I observation
- Experience in Long Term Care (LTC) or skilled nursing/rehab settings
- Ability to perform standardized assessments
- Ability to develop and document plan of care
- Ability to record treatment minutes per regulatory requirements
- Ability to provide wound care
- Ability to provide manual techniques
- Ability to provide therapeutic exercise
- Ability to provide ambulation training
- Ability to use modalities
- Ability to use assistive devices
- Ability to supervise and support PT assistants
- Ability to support rehabilitation aides
- Ability to support students
- BLS certification from AHA or ARC
Desired Qualifications
- Previous experience with electronic documentation systems in a skilled nursing or rehab environment
