Physical Therapist - Travel Contract

Job Summary
Provide skilled physical therapy services in a Skilled Nursing Facility via a travel contract, developing and implementing individualized treatment plans, collaborating with interdisciplinary teams to improve functional mobility and patient recovery, and contributing to quality improvement initiatives. Responsibilities include therapeutic exercise, gait training, and functional mobility interventions, with shift hours from 08:30 to 16:30 across five 8-hour shifts; benefits (medical, 401k matching, housing support, travel and license reimbursement) are available from day one.
Required Qualifications
- Bachelor's, Master’s, or Doctoral degree in Physical Therapy from an accredited program approved by the APTA
- Active Illinois state PT license required to start the assignment
- BLS certification from AHA or ARC may be required
