Physical Therapist - Travel Contract

Job Summary
Provide patient-centered home health physical therapy services to patients recovering from injuries and managing chronic conditions in home settings. Use Home Care Home Base EMR to document and coordinate care, work with interdisciplinary teams to develop and implement individualized care plans, and maintain documentation to meet productivity standards (30 visits per week). Role requires Illinois PT license, BLS (ASHA/ARC may be required), ability to travel to patient homes (approximately 25 miles per day), and participation in a 5-day orientation. Assignment is a travel contract based in Lisle/Naperville, IL with housing and travel/license reimbursement benefits.
Required Qualifications
- Bachelor’s, Master’s, or Doctoral degree in Physical Therapy from an accredited program approved by the APTA
- Active Illinois Physical Therapy license
- BLS certification may be required
- Ability to travel to patient homes
- Experience with Home Care Home Base EMR preferred
- Productivity expectation: 30 visits per week for registered therapists
