Physical Therapist - Travel Contract

Job Summary
Provide skilled physical therapy services in a traveling capacity within a skilled nursing facility in Park Ridge/Chicago area, focusing on restoring function and improving quality of life for geriatric/post-acute patients. Collaborate with nursing and interdisciplinary teams to create personalized treatment plans, document outcomes to support quality measures and resident satisfaction, and support rehabilitation across a five-day week (08:30–16:30, five 8-hour shifts). Requires graduation from an accredited PT program (A PT A), active IL PT license (or ability to obtain), and BLS; prior travel therapy and geriatric experience preferred.
Required Qualifications
- Graduation from an accredited physical therapy program (Bachelor’s, Master’s, or Doctoral degree) approved by the APTA.
- Active Illinois Physical Therapy license or ability to obtain state licensure prior to assignment start.
- Basic Life Support (BLS) certification from AHA or ARC may be required.
- Recent experience working in skilled nursing, long-term care, or post-acute rehabilitation settings preferred.
