Job Summary

Travel/Home Health Physical Therapist position located in Urbandale, IA. Deliver patient-centered care in patients' homes, perform assessments, recommend durable medical equipment, and collaborate with interdisciplinary teams. Responsibilities include conducting home visits, managing injuries and chronic conditions, providing patient education, and coordinating care across settings while traveling locally to patient homes. Requires a BS in Physical Therapy (or higher), Idaho PT licensure (or eligible), CPR, valid driver’s license, reliable transportation, 1–2 years of acute care/rehabilitation experience, English proficiency, and willingness to travel. Preferred: prior home health experience. Assignment is a travel/temporary contractor role in a home health setting.

Required Qualifications

  • Bachelor of Science in Physical Therapy from an APTA-approved program, or equivalent (Master’s or Doctoral degree acceptable)
  • Current State License as a Physical Therapist in Idaho, or ability to obtain prior to start of assignment
  • Proof of current CPR certification
  • Current driver’s license, reliable transportation, and vehicle insurance
  • 1–2 years of experience as a Physical Therapist in an acute care or rehabilitation setting
  • Ability to perform heavy lifting, bending, pulling, pushing, prolonged standing and walking as required for home visits
  • Ability to work a flexible schedule and travel locally to patient homes
  • Must read, write, and speak fluent English
  • Preferred: previous home health experience
