Job Summary
Travel Physical Therapist providing home health services in Cadillac, MI and surrounding communities. Work within an interdisciplinary home health team to help patients recover from injuries and manage chronic conditions. Requires an active Michigan PT license; familiarity with Home Care Home Base EMR; documentation and visit planning in a home health setting; mileage reimbursement and travel within roughly a one-hour radius of the hub city. Required Qualifications
- Bachelor‘s, Master‘s, or Doctoral degree in Physical Therapy from an accredited program.
- Active Michigan physical therapy license required to start the assignment.
- BLS certification from AHA or ARC may be required.
- Experience with Home Care Home Base EMR preferred.
- Productivity expectation: 30 points per week.
- Ability to complete documentation and visit plans in a home health setting.
- Reliable transportation and ability to travel within an approximately one-hour radius of the hub city based on referral distribution.
