Job Summary
Provide high-quality outpatient physical therapy services focused on safe, effective return to community activities. Complete comprehensive evaluations, create individualized treatment plans, document care in accordance with clinic, state, and federal guidelines, and participate in performance improvement efforts. Uphold professional codes of conduct and organizational safety expectations. Floating to other clinic locations as needed.
Required Qualifications
- Bachelor‘s, Master‘s, or Doctoral degree in Physical Therapy from an accredited program approved by the APTA
- Active Illinois state physical therapy license required to start the assignment
- BLS certification from the American Heart Association or American Red Cross may be required
- Experience in outpatient physical therapy required
- Experience with patient evaluation, goal writing, and developing functional treatment programs
- Ability to complete timely, accurate medical record documentation, including letters of medical necessity and charge entry
- Must adhere to state Practice Act, maintain professional licensure in good standing, and meet state-mandated continuing education requirements
- Willingness to float to other clinic locations as needed
