Physical Therapist - Travel Contract

Job Summary
Provide in-home skilled physical therapy services in the St. Petersburg, FL area. Evaluate patients, develop and implement individualized plans of care, deliver therapeutic interventions to improve mobility, strength, balance, and safety, and educate patients and caregivers on disease management and home exercise programs. Ensure accurate, timely, and compliant documentation supporting medical necessity and homebound status. Collaborate with physicians and the interdisciplinary team, communicate changes in condition, participate in care coordination, complete OASIS, adhere to agency policies, and supervise PTA services when applicable. Schedule typically M-F 8:00 AM–4:30 PM with a 40-hour weekly guarantee. Mileage reimbursed between visits; 100% housing availability and travel/license reimbursement included. Required Qualifications
- Bachelor‘s, Master‘s, or Doctoral degree in Physical Therapy from an accredited program approved by APTA
- Active Florida PT license required to start the assignment
- Home health experience required
- OASIS knowledge required
- Experience with HCHB electronic documentation required
- BLS certification from AHA or ARC may be required
