Job Summary
Travel Physical Therapist in home-health setting; conducts evaluations, interventions, and patient education across neuromuscular, neurological, and orthopedic conditions; documents Plans of Care and progress, teaches safe transfers and equipment use; collaborates with interdisciplinary team; maintains infection control practices; requires Ohio PT license, CPR, driver’s license; 1–2 years acute care/rehabilitation experience; familiarity with HCHB and OASIS; travel locally between Akron and Cleveland; heavy lifting and flexible schedule; on-call; service areas include Cuyahoga, Summit, Stark, Lake, and Medina counties; Solon, OH location with remote travel between markets; benefits include medical, 401(k), housing options, and license reimbursement.
Required Qualifications
- Current State Physical Therapist license required for Ohio prior to start
- Bachelor of Science, Master’s, or Doctoral degree in Physical Therapy from an APTA-approved program
- Proof of current CPR certification required
- Current driver’s license, reliable transportation, and vehicle insurance required
- 1–2 years experience as a Physical Therapist in acute care or rehabilitation settings required
- Experience with HCHB, OASIS, and home health documentation systems required
- Ability to perform heavy lifting, bending, pushing, pulling, prolonged standing and walking
- Ability to work a flexible schedule and travel locally
- Floating to nearby branch coverage may be required
