Job Summary
Provide high-quality physical therapy care to residents in skilled nursing settings. Develop and implement individualized treatment plans to support rehabilitation goals, focusing on mobility training and post-acute care. Collaborate with an interdisciplinary team and maintain thorough documentation to support safe discharge and functional gains. This is a travel contract based in Sauk City, WI requiring Wisconsin licensure and with benefits including housing assistance and license reimbursement where applicable. Immediate submissions of resumes are requested; BLS may be required.
Required Qualifications
- Degree in Physical Therapy from an accredited program approved by the APTA
- Active Wisconsin Physical Therapist license
- BLS certification from the American Heart Association or American Red Cross (may be required)
- Experience in skilled nursing or long-term care settings preferred
- PT or PTA candidates accepted
