Job Summary
Travel Physical Therapist in Tillamook, OR providing goal-directed physical therapy interventions that support patient independence and safety in the home. Collaborate with interdisciplinary teams to develop and implement individualized care plans, contribute to quality outcomes through documentation, patient education, and family/caregiver training. Position involves travel to patient homes within the service area and use of a personal vehicle for visits; shift is Monday–Friday, 40 hours/week on a day schedule.
Required Qualifications
- Doctor of Physical Therapy (DPT) degree, or foreign equivalent. (DPT required if license obtained on or after January 2016.)
- Active Physical Therapy license valid in the applicable state(s) of practice
- Home health experience
- Valid driver’s license and minimum age of 21 years
- CPR or BLS certification
- Availability to work a 40-hour workweek on day shift
