Job Summary
Conduct home health physical therapy services for patients recovering from injuries and managing chronic conditions in Shelbyville, KY. Complete SOCs/ROCs, routine visits, supervisory visits, and re-assessments while documenting in OASIS and HCHB to ensure quality outcomes and continuity of care. Work Monday–Friday, 8:00 AM–4:30 PM, with 32 weekly guaranteed hours and no on-call or weekend requirements. Mileage reimbursement covers travel between visits.
Required Qualifications
- Bachelor's, Master's, or Doctoral degree in Physical Therapy from an accredited program approved by the APTA
- Active Kentucky Physical Therapist license
- Home health experience
- Experience with OASIS and HCHB
- BLS certification from AHA or ARC
- Must be able to complete SOCs/ROCs, routine visits, supervisory visits, and re-assessments
