Job Summary
Provide skilled evaluations, supervisory visits, and ongoing therapy visits to patients recovering from injuries or managing chronic conditions across Kitsap County. Document all care using Wellsky and OASIS, collaborating with nurses for SOC documentation while meeting a weekly productivity expectation of 30 points. Work flexible schedules including 5x8s or 4x10s with standard mileage paid and higher rates for travel over 45 miles. No weekend rotation or on-call requirements apply.
Required Qualifications
- Bachelor's, Master's, or Doctoral degree in Physical Therapy from an accredited program
- Active Washington State PT license
- BLS certification from AHA or ARC
Desired Qualifications
- Experience with home health settings
- Experience using Wellsky and OASIS documentation
- Familiarity with OASIS SOC, ROC, Recert, and Discharge processes
- Prior experience covering territory-based caseloads
- Comfort with productivity expectations measured in OASIS and visit points
- Experience collaborating with nurses who complete portions of OASIS SOC documentation
