Job Summary
Conduct outpatient physical therapy services to help patients recover from injuries and manage chronic conditions. Evaluate patients, develop and implement individualized treatment plans, and perform documentation including goal writing and letters of medical necessity. Utilize functional treatment programs and safe patient handling techniques, while supervising or directing support staff and students as needed. Maintain professional licensure and meet state-mandated continuing education requirements. This 13-week, 5x8 shift assignment in South Bend, IN, offers proximity to Notre Dame, the Indiana Dunes, and family-friendly attractions like the Potawatomi Zoo.
Required Qualifications
- Bachelor's, Master's, or Doctoral degree in Physical Therapy from an accredited program approved by the APTA
- Active Indiana Physical Therapy license
- Outpatient physical therapy experience
- Experience evaluating patients
- Experience developing and implementing individualized treatment plans
- Experience performing documentation including goal writing
- Experience writing letters of medical necessity
- BLS (Basic Life Support) certification from AHA or ARC
Desired Qualifications
