Job Summary
Conduct physical therapy services for patients recovering from injuries and managing chronic conditions within San Bernardino County. Maintain daily documentation using the HCHB EMR system while adhering to a Monday through Friday 8 a.m. to 5 p.m. schedule with no on-call or weekend requirements. Hold a Bachelor's, Master's, or Doctoral degree in Physical Therapy from an APTA-accredited school, an active state license, and at least one year of home health experience, including OASIS certification.
