Job Summary
Provide skilled, individualized home-based physical therapy for patients recovering from injury or managing chronic conditions in Alpena, MI. Conduct evaluations, therapeutic interventions, and discharge planning while collaborating with nurses, occupational therapists, and family caregivers. Document clinical progress in HCHB electronic medical records to support patient-centered plans of care and continuity. Work a Monday through Friday schedule of 40 hours per day, with 8-hour shifts covering multiple ZIP codes in a rural service area.
Required Qualifications
- Bachelor's, Master's, or Doctoral degree in Physical Therapy from an accredited program approved by the APTA
- Active Michigan Physical Therapy license
- At least 12 months of home health physical therapy experience within the past 3 years
- Experience with HCHB (Homecare Homebase) electronic medical record
- BLS certification from AHA or ARC
- Valid driver's license
- Reliable transportation to travel to patient homes within the assigned territory
Desired Qualifications
- Prior travel or per-diem home health experience
- Experience managing caseloads across multiple ZIP codes within a rural service area
