Job Summary
Provide rehabilitative services to older adults in skilled nursing and senior living communities, delivering evaluations, individualized treatment plans, and progress documentation. Collaborate with interdisciplinary teams to improve patient outcomes while managing daily volumes of 8–15 patients consistent with an 85% productivity standard. Required Qualifications
- Bachelor's, Master's, or Doctoral Degree in Physical Therapy from an accredited program approved by the APTA
- Active Minnesota Physical Therapist license
- Minimum 6 months of recent experience in a skilled nursing facility (SNF) or similar post-acute setting
- Experience managing daily patient volumes consistent with SNF expectations, including productivity standards
Desired Qualifications
- BLS certification from AHA or ARC
- Experience with high daily patient volumes (approximately 8–15 patients per day)
- Familiarity with productivity standards and documentation expectations in long-term care or senior living environments
- Previous travel or contract therapy experience
