Job Summary
Provide skilled home health physical therapy services supporting patient independence and recovery in Zanesville, OH. Deliver evidence-based assessments, individualized treatment plans, and patient education while coordinating care with an interdisciplinary team. Document outcomes using required home health systems and manage a caseload of 32 points per week across Perry and Morgan counties. Work Monday through Friday, 40 hours per week, with 8-hour shifts and on-call rotation for travel within the assigned territory. Required Qualifications
- Bachelor's, Master's, or Doctoral degree in Physical Therapy from an accredited program approved by the APTA
- Active Ohio Physical Therapy license
- BLS certification from AHA or ARC
- Home Health experience
- Experience with HCHB (Homecare Homebase)
- Availability to work within the assigned territory covering Perry and Morgan counties
- Monday through Friday, 40 hours per week
- 8 hours per day
- 32 points per week
