Physical Therapist - Travel Contract

Job Summary
Conduct outpatient physical therapy services by evaluating patients, developing individualized treatment plans, and documenting care in compliance with regulatory and clinic standards. Supervise discipline-specific therapy services, participate in student education and professional development, and complete timely medical record documentation including letters of medical necessity. Adhere to safety procedures, state Practice Act, and organizational codes of conduct while maintaining licensure in good standing. Float to other clinic locations as needed. This assignment includes a 5x8 schedule, 100% paid housing, and travel reimbursement.
Required Qualifications
- Bachelor's, Master's, or Doctoral degree in Physical Therapy from an accredited program approved by the APTA
- Active Illinois state physical therapy license
- BLS certification from the American Heart Association or American Red Cross
- Experience in outpatient physical therapy
- Experience with patient evaluation, goal writing, and developing functional treatment programs
- Ability to complete timely, accurate medical record documentation, including letters of medical necessity and charge entry
- Willingness to float to other clinic locations as needed
Desired Qualifications
- Experience supervising students and participating in clinical education programs
- Prior experience in performance improvement initiatives and familiarity with competency programs for direct care providers
- Demonstrated safe patient handling and materials handling practices
