Job Summary
Conduct physical therapy services for patients recovering from injuries and managing chronic conditions across Providence, Bristol, Kent, and Washington counties. Deliver home exercise planning and OASIS documentation while maintaining a productivity target of 30 points. Complete a weekend requirement of two days per month in addition to standard Monday through Friday coverage from 8:00 AM to 5:00 PM. Hold an active state license, BLS certification, and HCHB homecare experience to begin the assignment.
Required Qualifications
- Bachelor's, Master's or Doctoral Degree in Physical Therapy from accredited schools approved by the APTA
- Active State License
- HCHB Homecare experience
- Home exercise planning experience
- OASIS experience
- RI compact license
- 2 weekend days per month
- 30 points productivity requirement
Desired Qualifications
- BLS Certification from AHA or ARC
