Job Summary
Deliver home health physical therapy in Marion, OH, helping patients recover from injuries and manage chronic conditions. Perform evaluations, implement treatment plans, and document outcomes using HCHB and OASIS systems to improve patient function and quality of life. Maintain a guaranteed 40-hour work schedule with 8-hour shifts from Monday through Friday, meeting productivity expectations of 30 points per week. Work within interdisciplinary teams to support recovery and independence while experiencing life in a new location.
Required Qualifications
- Bachelor's, Master's, or Doctoral degree in Physical Therapy from an accredited program approved by the APTA
- Active Ohio Physical Therapy license
- BLS certification from AHA or ARC
- At least 12 months of home health experience within the past 3 years
- Experience with HCHB and OASIS documentation systems
- Ability to meet productivity expectations of 30 points per week
- Ability to work a guaranteed 40 hours per week
Desired Qualifications
- Previous experience working with complex patient caseloads in home health settings
- Previous travel clinician experience
