Job Summary
Treat orthopedic, neuro, and safety discharge patients in home health settings while supporting continuity of care between outpatient and acute facilities. Manage patient recovery from injuries and chronic conditions as part of an interdisciplinary team. Work four days in outpatient clinic and one day in acute care, with potential weekend rotations and occasional on-call coverage. Required Qualifications
- Bachelor's, Master's, or Doctoral degree in Physical Therapy from an accredited program approved by the APTA
- At least 24 months of current Physical Therapist / PT experience
- At least 1 year of previous PT experience
- Active State Physical Therapy license
- BLS certification from AHA or American Red Cross
- Driver's license
- EPIC electronic medical record experience
- Outpatient experience
- Work history/resume/employment history documenting the required 24 months of PT experience
- Skills checklist specific to Physical Therapist discipline
Desired Qualifications
- Candidates without an Iowa license willing and able to apply for the Iowa license
- Inpatient experience
- Travel experience: at least one completed travel contract
- Professional reference or performance evaluation from a charge/supervisor/nurse manager/director/CNO/COO within the past 24 months
- Previous acute care weekend experience
- Experience with orthopedic and neurological patient populations
