Job Summary
Conduct patient-centered rehabilitation in a skilled nursing facility to support functional recovery, fall-risk reduction, and discharge planning for mixed caseloads including high-acuity and geriatric patients. Manage chronic conditions and injuries while participating in interdisciplinary care teams to optimize outcomes for older adults. Requires an active Wisconsin state license, BLS certification, and experience in skilled nursing, assisted living, or memory care settings. Schedule includes M–F days and one weekend per month. Required Qualifications
- Bachelor's, Master's, or Doctoral degree in Physical Therapy from an accredited program approved by the APTA
- Active Wisconsin state license or compact license recognized for practice prior to the start of the assignment
- BLS certification from AHA or ARC
- Experience working in skilled nursing facility, assisted living, independent living, or memory care settings
Desired Qualifications
- Experience using NetHealth (electronic medical record)
- Previous travel therapy experience
- Experience managing mixed caseloads including high-acuity and geriatric patients
