Job Summary
Provide skilled physical therapy services in a skilled nursing environment, delivering individualized care plans that support functional mobility, fall reduction, and ADL independence. Collaborate with interdisciplinary teams to optimize patient outcomes and continuity of care while managing typical skilled nursing documentation and care planning systems. Work onsite at the Farmington skilled nursing facility with a schedule of five 8-hour shifts from 08:30 to 16:30, with potential to float within the local service area if needed.
Required Qualifications
- Bachelor's, Master's, or Doctoral degree in Physical Therapy from an accredited program approved by the APTA
- Active Illinois Physical Therapy license
Desired Qualifications
- BLS certification from AHA or ARC
- Experience working in a skilled nursing facility or long-term care setting
- Ability to manage typical skilled nursing documentation and care planning systems
- Previous travel therapy experience
- Experience with mobility training, restorative programs, and ADL-focused interventions
