Job Summary
Conduct independent physical therapy for home health patients recovering from injuries or managing chronic conditions within a defined service territory covering San Jose and neighboring cities. Manage caseloads to meet productivity expectations of at least 27 weekly points while working day shifts. Travel within the assigned county and occasionally outside the area to secure full caseloads. Weekend coverage may be required. Join a travel PT program offering schedule flexibility and professional variety across diverse home health environments. A recruiter will provide details on credentialing, onboarding, and specific assignment logistics.
Required Qualifications
- Graduate of a CAPTE-accredited Physical Therapy program (Bachelor's, Master's, or Doctoral degree)
- Active California Physical Therapist license
- BLS certification from AHA or ARC
Desired Qualifications
- Experience in Home Health settings
- Willingness to travel within assigned county territory (Area #12 San Jose and Area #11 including Santa Clara, Sunnyvale, Cupertino, Milpitas, Mountain View, Campbell, Los Altos, Saratoga, Los Gatos)
- Willingness to work weekends when needed
- Ability to manage a caseload and meet productivity expectations
- Client notes indicate expectation of at least 27 weekly productivity points as a performance target
