Job Summary
Home Health Physical Therapist to provide skilled therapy in patients' homes, delivering individualized care plans, working with an interdisciplinary team to improve strength, mobility, and independence. Full-time, Monday–Friday, 8:00am–4:30pm, with no holidays or weekends. Competitive hourly wage of $55-$65, with benefits including company car, health/dental/vision, 401k with match, FSA, tuition reimbursement, PTO, and orientation/training. Independent work in a home-care setting within Western Wisconsin, serving Sauk City area.
Required Qualifications
- Active Wisconsin Physical Therapist License
- CPR Certification
- A valid Wisconsin Driver's License and Car Insurance
