Job Summary
Develop and revise individualized care plans based on physician orders while performing skilled physical therapy procedures to help patients regain strength and mobility at home. Educate patients and families on daily home programs and collaborate with physicians, mentors, and the interdisciplinary team to support recovery. Participate in IDT meetings, in-service training, and quality assurance initiatives. Requires an active Wisconsin Physical Therapist license, CPR certification, and valid driver's license. Flexible daytime hours from 8:00 AM to 4:30 PM, Monday through Friday, with no holidays or weekends required.
Required Qualifications
- Active Wisconsin Physical Therapist license
- CPR certification
- Valid WI driver's license & car insurance
- Comfortable working independently in a home care environment
