Job Summary
Provide one-on-one physical therapy to older adults in-home or in Senior Living Communities, including evaluations, individualized plans of care, and evidence-based treatment; deliver therapy to improve mobility, independence, and aging-in-place; document in FOX’s EMR; collaborate with an interdisciplinary team (PT, OT, SLP, Fitness Specialists); educate patients, caregivers, and staff; telehealth/virtual care optional to extend access; flexible schedule with option for in-home and community-based care; H1B sponsorship available for qualified clinicians; new graduates welcome to FOX’s Emerging Professionals Mentor Program; salaried roles available (Full-time, Part-time, or PRN/Flex) with benefits; requirements include PT license, degree, CPR, and EMR proficiency.
Required Qualifications
- Valid Physical Therapy license in the state(s) of practice, or eligibility to obtain
- Degree from an accredited Physical Therapy program
- Basic computer and EMR proficiency
- Current CPR certification
