Assess, plan, and participate in rehabilitative programs to improve mobility, relieve pain, and increase strength for patients with disease or injury. Implement treatment plans, select appropriate activities to progress patients toward goals, and revise plans with patient and family input. Provide ongoing education, discharge instructions, and referrals to community resources. Collaborate with physicians to establish and evaluate treatment programs, and document evaluations, sessions, and care plans according to departmental guidelines. Adhere to ethical standards, infection prevention precautions, and system policies while fostering teamwork and participating in staff meetings.
