Assess, plan, and organize rehabilitative programs to improve mobility, relieve pain, and increase strength for patients with disease or injury. Implement treatment plans, select appropriate activities, and revise goals with patient and family input. Provide ongoing education, discharge instructions, and referrals for community resources. Collaborate with physicians to establish evaluation and discharge plans, while documenting sessions and care according to departmental protocols. Adhere to ethical business practices, infection prevention precautions, and system policies. Maintain punctuality and participate in staff meetings and process improvement activities. Requires a Bachelor's, Master's, or Doctorate in Physical Therapy.
