Job Summary
Provide 1:1 patient care by evaluating physical, functional, and cognitive status to determine safe capabilities. Document all evaluations, treatments, and care coordination contacts while independently managing your case load to maximize effectiveness. Deliver education and counseling to patients and families regarding treatments, goals, and safety risks within your scope of practice. Comply with all applicable federal, state, local laws, regulations, and facility-specific policies. Support special projects and designated activities as assigned by leadership. Join a two-year PT Cohort Program featuring collaborative learning, curated mentorship, and a roadmap for career growth.
Required Qualifications
- Physical Therapist license OR anticipated completion of DPT coursework and exams within 6 months
