Job Summary
Provide one-on-one physical therapy to older adults in-home (private residences) and within senior living communities, performing evaluations, developing individualized plans of care, and delivering evidence-based treatment. Deliver therapy to support mobility, independence, and aging-in-place goals; document in FOX’s EMR; collaborate with an interdisciplinary team (PT, OT, SLP, Fitness Specialists); educate patients, caregivers, and staff; offer telehealth/virtual care when appropriate; maintain autonomy to create and adjust your treatment schedule; must travel to patient homes and comply with PPE requirements. Required Qualifications
- Valid Physical Therapy license in the state of practice
- Degree from an accredited Physical Therapy program
- CPR certification
- EMR proficiency
