Job Summary
Evaluate residents and develop individualized physical therapy treatment plans with short- and long-term rehabilitation goals. Provide skilled therapy services to improve mobility, strength, balance, and functional independence while supervising Physical Therapist Assistants. Complete required documentation, evaluations, care plans, and billing records accurately. Collaborate with physicians, nursing staff, and families to support resident outcomes, recommend durable medical equipment, and coordinate discharge planning. Maintain therapy equipment and participate in care conferences and quality improvement initiatives. Work full-time, 8 am to 4 pm, at Roscoe Rehabilitation & Nursing Center in the Catskill Mountains.
Required Qualifications
- Master's or Doctorate Degree in Physical Therapy from an accredited program approved by the American Physical Therapy Association
- Valid New York State Physical Therapist License
