Job Summary
Evaluate residents and develop individualized physical therapy treatment plans with short- and long-term rehabilitation goals. Provide skilled therapy services to improve mobility, strength, balance, and functional independence while supervising Physical Therapist Assistants. Complete required documentation, evaluations, care plans, and billing records accurately. Collaborate with physicians, nursing staff, families, and interdisciplinary team members to support resident outcomes. Provide resident and family education regarding therapy goals, adaptive equipment, and home exercise programs. Recommend and coordinate durable medical equipment and discharge planning needs. Maintain therapy equipment, supplies, and treatment areas. Participate in care conferences, staff education, orientation, and quality improvement initiatives.
Required Qualifications
- Master's or Doctorate Degree in Physical Therapy from an accredited program approved by the American Physical Therapy Association
- Valid New York State Physical Therapist License
