Job Summary
Develop individualized therapy programs through physical assessment, treatment planning, implementation, education, and communication to maximize patient progress toward functional goals. Provide direct patient care by evaluating functional needs and outcomes, adjusting care plans, and directing support staff to achieve specific objectives. Design and implement plans of care that incorporate participation by significant others while maintaining positive relationships with other health professionals and ancillary staff. Conduct therapy practice within defined standards of care, document evaluations and treatment goals, and adhere to facility policies regarding safety and infection control. Maintain current licensure and proficiency with state and federal standards while engaging in educational programs to enhance professional growth.
Required Qualifications
- Knowledge and skills to care for patient needs of the SNF population
- Must possess problem solving skills to successfully perform job
- Excellent verbal and written communication skills
- Excellent organizational skills and attention to detail
- Strong customer service orientation
- Must be able to communicate effectively and professionally with other employees, patients, and family members
- Ability to travel as needed to perform job
- Graduate of an accredited Bachelors, Masters, or Doctorate in Physical Therapy
- Proficiency with all applicable state, federal, and professional standards
- Current license from the state Board of Physical Therapy Examiners
- Duties require extensive standing and walking
- Requires lifting, positioning, pushing, and/or transferring patients
- Duties require frequent reaching, stooping, bending, kneeling, and crouching
