Job Summary

Provide physical therapy services in home health settings, delivering one-on-one care to help patients recover, manage chronic conditions, and maintain independence in their homes. Responsible for evaluating patient function, developing individualized care plans, educating patients and caregivers, translating prescribed exercises into functional activities of daily living, recommending home and equipment modifications for safety, collaborating with physicians and an interdisciplinary team, documenting clinical notes and progress, motivating patients, and supervising Physical Therapist Assistants in accordance with policy and regulations.

Required Qualifications

  • Graduate of an accredited Physical Therapy program approved by the American Physical Therapy Association (APTA)
  • Current Physical Therapist (PT) license in the state of practice
  • Current CPR and First Aid certification
  • Reliable transportation, valid driver�s license, and current automobile insurance
